(Screenshots are images, and not functional).
- Greeting by CEO/President. The intent of the Introduction is to give a clear indication of CEO support for the survey, and to reassure participants that the results are going to an outside contractor.
- Survey Instructions. The purpose is to provide a brief overview of the survey format.
- Response-Sensitive Survey Flow. This is an example of a group of questions that is only asked of employees with less than one year's experience. Web-based surveys can tailor the survey flow to ask different populations distinct niche questions.

- Executive Summary provides a somewhat detailed top-level analysis in terms of five dimensions. This particular survey is sorted by (1) location and (2) employee tenure.
